Can someone explain to me how this pad has failed?
The small triangle next to it is a piece that crumbled off when I removed the pad from the calliper.
It is a bendix pad and from the rear of a commodore.

I'm only guessing here, but perhaps the pad flexed? It looks as though the end of the pad closest to the camera flexed, causing the pad material to crack and suffer a failure.
That or the pads were hot and you drove through some water? That would also stuff the rotors too. How are the rotors? What is the opposite pad like?

^ Rotors are ok. Opposite paired pad looks ok-ish... Opposite side (previous owner) was not changed at the same time and was worn out.

^^ the pictured pad was opposite the piston, on the floating side.

incorrect installation, maybe they have been used in an emergency and caused too much heat - thermal expantion - crack.

looks like someone pushed the piston back with a screwdriver against the pad
