Using the nuts is usually fine, the problem with installing the pins is that its far too easy to put the pin in slightly crooked so it doesn't seat right, you just end up mashing everything. The simple solution is as Heath says, get the cotter pin seated as far as possible by hand wiggling things around, then you know its straight before putting load on the thread.
Don't try and "pull" them tight with the nut, I've stripped one doing it that way and that was a huge pain in the butt.
This could also be an issue using the tool I made to remove the pins.
