Did all LX models come with the three roof grab handles?

I believe the answer is yes. No cars had them where the driver sits.

 

I only half understand why they omit them for the driver's seat.

Yeah the driver doesn't need a Jesus strap.

I imagine that some less physically able (elderly etc.) consumers would still sometimes have want for a jesus strap for the driver's position. They don't get rid of this kind of thing on 4WD's I notice.

Ah! My bad. Base model ('S' trim level) cars had these by the rear seats and a grab handle by the front passenger (only) instead:
